The initial assessment session is $160.00.The rate for individual, family, couples counseling or educational consultation is $150. I also accept clients on a sliding scale at this time. Please call me to discuss your particular financial needs.
I am a provider on the following insurance panels:
Blue Cross Blue Shield of Texas
I am out-of-network on other panels at this time. As a professional courtesy to you, I can provide an invoice for the services that you receive and the payments that you have made so that you can submit these to your insurance company for reimbursement. Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session in-network and out-of-network?
- Is approval required from my primary care physician?
You will be responsible for seeking reimbursement and filing claims.
*Please note that insurance REQUIRES a diagnosis and this diagnosis becomes a part of your permanent medical record. In many instances, family and couples counseling may not be covered by your insurance plan. If would like to know about receiving services without using insurance, please contact me.
Reduced fee services are available on a limited basis. Please contact me if you are interested in my services but cannot afford the current rates. I am happy to discuss your situation with you personally to see if I can be of service to you.
Payment is accepted at the beginning of each therapy session. Cash, check and all major credit cards accepted for payment.
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay for the session in full. If you no-show a scheduled appointment, you will be charged the full fee for that session.
Melissa McDavid LMFT, LPC, LSSP, NCSP makes every attempt to run on schedule as to not interfere with the next scheduled appointment. Consequently, if you are more than 15 minutes late, your appointment will be considered a “no show”. She has the right to ask a client not to return for services in the event that the client “no shows” or cancels for two consecutive appointments. In that case, referrals will be provided for the client to seek services with another provider if desired.
